Coverages, Policy Forms, and Producer Forms
Missouri FAIR Plan provides basic insurance coverage for dwellings, commercial property, and farm property. YOU SHOULD CAREFULLY READ ALL POLICY FORMS TO DETERMINE ACTUAL COVERAGE.
General Forms
Dwelling
To qualify as a dwelling, a property can contain one to four families. We provide a DP 00 01 named peril policy which includes, fire, lightning, explosion, windstorm, hail, riot or civil commotion, aircraft, vehicles, smoke, volcanic eruption, vandalism and malicious mischief. Theft coverage may be added to the dwelling policy as a separate content item. The limits for theft coverage are a minimum of $1,000 and a maximum of $25,000. Contents along with extended coverage and vandalism and malicious mischief must be included to be eligible for theft coverage. The maximum we offer is $200,000 on buildings, contents and theft combined. This coverage is available for both owner and tenant occupied risks.

Commercial
To qualify as commercial, a property can contain five or more families or used for business purposes. We provide a Standard Property Policy CP 0099 named peril policy which includes fire, lightning, explosion, windstorm, hail, riot or civil commotion, aircraft, vehicles, smoke, volcanic eruption, vandalism and malicious mischief. The maximum we offer is $1,000,000 on all buildings and contents combined at one location.

Farm
To qualify as a farm, a property must have farm operations that generate a profit. A house with acreage is not a farm. We provide a FP 0012 and FP 0014 that cover the dwelling, the contents of the dwelling and the outbuildings. THERE IS NO COVERAGE FOR FARM EQUIPMENT, CROPS, LIVESTOCK OR LIABILITY.
The named peril policy provides fire, lightning, windstorm, hail, and explosion. riot or civil commotion, aircraft, vehicles, smoke, vandalism and limited theft. The maximum we offer is $1,000,000 on all buildings and contents combined at one location.
Sinkhole Loss
To qualify as a dwelling, a property can contain one to four families. We provide MFP-SL-2 standalone sinkhole actual cash value policy. The maximum we offer is $200,000 on building and contents combined. This coverage is available to the owner of both owner and tenant occupied properties. As stated in the law only property or personal property owned by the applicant / owner of the property may be covered. WE DO NOT OFFER A CONTENTS ONLY SINKHOLE LOSS POLICY.

Producer Forms and Instructions
The Missouri Fair Plan application must be completed for dwelling, commercial or farm property. If completing a commercial application, you must also submit a Class Rate form and a Statement of Diligent Effort. If completing a farm application, you must also submit a Farm Property form and a Statement of Diligent Effort.
For vacant properties, the producer must also complete the Vacancy Supplement Information Form and submit with the application.
There is a separate sinkhole application required for this coverage. Submit an application for each fire division or habitational structure. Along with the application you will find the sinkhole rate schedule. The premium for the coverage desired should be submitted with the completed application and photos.
Front and rear photos must be submitted with all applications. Include contents photos if applying for contents coverage.
